Humana - - Responsibilities: Provide direct patient care to patients under the direction of the RN and according to the Aide Plan of Care (POC).; Assist the patient with self-administered medications and monitor for proper administration.; Take accurate vital signs and report abnormalities to supervisor.; Assist with personal hygiene and activities of daily living.; Assist with feeding and nutrition observation; support self-feeding as needed.; Assist with toileting and ostomy bag management as applicable.; Support patient mobility, positioning, transfers and ambulation per Plan of Care.; Document care interactions and changes for continuity of care.; Maintain infection control and provide a safe, clean home environment.; Assist with light household tasks as needed with MD order present.
