CenterWell Senior Primary Care - - Responsibilities: Provide direct patient care to patients under the direction of the RN and according to the Aide Plan of Care; Take accurate temperature, pulse and blood pressure measurements and report abnormal results; Help patient maintain personal hygiene and assist with bathing, grooming, oral care, and related activities; Assist in feeding patients and observe/record food and fluid intake; Assist with patient mobility, transfers and ambulation per Plan of Care and policy
