Sevita - - Responsibilities: Deliver support per individual service/program plans, including socialization and behavioral development, personal care, housekeeping, transportation, community orientation, shopping, citizenship, and other activities of daily living.; Accurately documents progress and activity; reviews records and logs to stay abreast of changes in service plans; maintains confidentiality.; Maintain health care activities such as accompanying to medical appointments, relaying orders, administering or supporting self-administration of medications, and monitoring health concerns.; Perform meal preparation and assist with daily meals according to planned menus and dietary requirements.; Foster teamwork, communicate openly, and ensure a safe workplace while following safety policies.
