Position Summary:

The Part-time Registered Nurse Case Manager will report directly to the Director of Nursing. They will help ensure that our clients receive safe, professional, and loving care in their homes. The Part-time Registered Nurse Case Manager will complete in-home clinical assessments, provide supervision of aides and provide guidance and training to caregivers to ensure the highest quality of care for our patients.

Key Responsibilities:

• Conduct In Person patient interviews and comprehensive physical assessments.

• Oversee the implementation and ongoing assessment of the patient's plan of care through the management of home health aides, LPNs, RNs, and other caregivers.

• Communicate patient conditions and collaborate with appropriate providers to deliver care when patient needs evolve.

• Provide education to patients and families on proper home health care procedures. Ie. Wound care, IV administration, medication management.

• Work to decrease readmissions by promoting preventive care and ensuring continuity of care.

• Document patient information accurately and maintain medical records.

• See patients and clients, as needed.

• Auditing charts weekly.

• This position requires frequent standing, walking, bending, and lifting up to 50 lbs, with occasional assistance in patient transfers.

• Perform other duties as assigned.

Occupational Exposure Risk:

• Category I -Routine Exposure

Physical and Environmental Requirements:

• Frequent walking and standing

• Ability to drive and make occasional home visits

• Ability to climb stairs, must be able to squat, kneel, bend and reach

• Ability to lift up to 50lbs with occasional assistance in patient transfers

• Exposure to bloodborne pathogens and infectious materials is expected; appropriate PPE training will be provided.

• Maintain confidentiality as related to client information under HIPAA regulations regarding Protected Health Information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offense.

Education:

• Associate or Bachelor's degree in a Nursing (Required).

Qualifications:

• Unrestricted Licensed IN Registered Nurse (RN) is required.

• Current CPR/AED/BLS/First Aid certification.

• One (1) year of professional experience practicing as a Registered Nurse (RN) in home health; previous case management/utilization review experience preferred.

• Must be able to drive regularly, climb stairs, and tolerate varied home environments (e.g., temperature, allergens)

• Must have an active driver license and reliable transportation to drive throughout the day / territory.

• Fine motor skills, visual acuity, and auditory perception are required for patient assessments and documentation.

• Extensive OASIS experience and knowledge with a demonstrated ability to oversee and ensure compliance with OASIS guidelines.

• Ability to effectively communicate with patients, families, and healthcare professionals.

• Proficient in conducting physical examinations and assessing patient conditions.

• Strong knowledge of medical terminology and pharmacology.

• Ability to interpret medical test results and develop appropriate care plans.

• Exceptional critical thinking and problem-solving abilities.

• Medicare, ACHC and/ or CHAP experience preferred.

• US work authorization (Required)

• Must have your own computer and high-speed internet. We use MS Teams for regular video calls.

Job Type: Part-time

Salary: $40-$45/ hour

Schedule:
